NB:

Provision relates to the general and geographical distribution of interventions, and is rated as: full, provided in nearly all relevant locations (areas in which the target population is sufficient for implementation of the intervention); extensive, provided in a majority, but not all relevant locations; limited, provided in more than a few, but not the majority of relevant locations; rare, provided in just a few relevant locations; none, intervention not available. Information was collected by means of a structured questionnaire.
